Planning List Administrator - Region 6

Job Description

Planning List Administrators are assigned to specific counties that they are responsible for and, under general supervision, will perform the following duties and responsibilities for people with MR/DD in these counties.


Duties and Responsibilities include:
  • Assist persons on planning list to access state plan services, stated funded DD services and advocate for those with most in need, as defined in the planning list policy, for Medicaid waiver services
  • Participate, as needed, in the regional planning list meetings to advocate for people with DD who reside in assigned counties
  • Assist people with DD and their families with the Medicaid application process; when person is approved for the Medicaid waiver coordinate process to obtain the correct Medicaid category
  • Assist people with DD who are eligible for Medicaid to gain and coordinate access to necessary care and services to meet their needs which may include medical and social services
  • Assist people with DD to schedule appointments with Medicaid participating physicians and arrange for transportation to appointments
  • Assist people with DD to apply for SSI and provide advocacy in appeals
  • Provide assistance to person with DD once they have been approved for a Medicaid waiver to select a provider by assisting them in choosing a service provider either by facilitating meetings with the potential providers and scheduling provider fairs as warranted and reporting weekly status to the regional Intake and Evaluation staff
  • Maximize the health and safety of people with DD by maintaining close contact with those who have significant health needs and assist them and their families to get the support needed through the state plan
  • Be responsible for the administration of Health and Human Rights Committees in assigned counties to ensure the health needs of all persons with DD are being met
  • Assist in transitioning people with DD from hospital to community placements and transition to support coordination
  • Establish working relationships with local state agencies to include Family and Children services, Babies Can't Wait and Aging services

 

Qualifications

Bachelors degree in a related field and two years of related experience OR Masters degree in a related field


Note:  Some positions require eligibility for licensure

 

Preferred Qualifications

Professional experience working with individuals with developmental disabilities


Experience with transitions to a community waiver service

Strong computer skills

Strong communication and organizational skills

Ability to work independently

This position is located in the Region 6 Office in Columbus, GA. Region 6 is comprised of the following counties: Carroll, Heard, Coweta, Troup, Meriweather, Fayette, Henry, Butts, Spalding, Pike, Lamar, Upson, Crawford, Peach, Houston, Taylor, Macon, Dooly, Crisp, Marion, Schley, Sumter, Webster, Harris, Talbot, Muscogee, Chattahoochee, Stewart, Quitman, Randolph and Clay.

Columbus, Georgia is an authentic Rivertown. The gently rolling waters of the Chattahoochee have provided food, energy, employment, and recreation for the citizens of Columbus throughout the years.

Today, the river is home to the 12-mile path known as the Chattahoochee Riverwalk. The Riverwalk provides an excellent facility to bike, in-line skate, jog, or just take a leisurely stroll and enjoy the scenery. The river is also home to the Chattahoochee Princess Riverboat, an authentic 1880's era paddlewheel riverboat, and the Coca-Cola Space Science Center, the first in Georgia and one of only 32 in the United States.
